Volunteers who want to drive on a field trip or be overnight chaperones need a live scan.


For information about where to get a live scan done, go here:  To find out if a live scan you had done recently is still valid, contact  If you use the form from Oakland Ed Fund (see below), we have been told you won’t have to do it for OUSD again.

Here is the TB risk assessment form for people who think they are at low risk of having TB.  It needs to be renewed every 4 years and is required for all school volunteers coming on a regular basis.tbcb-adult-tb-risk-assessment-for-schools

If you plan to drive on a field trip, you will need to bring a copy of your driver’s license, up-to-date auto insurance, and this completed form to the school office.  If you have students at other schools, please bring the same documents to that school site if you plan to drive on a field trip there. driver-form
